To use downloaded form please print three copies of .pdf form, mark one copy "WHITE", second copy "BLUE" and third copy "YELLOW" and fill ALL copies in in full.

PLEASE NOTE: That both the blue AND white forms should be returned with your entry (not blue only as stated in newsletter) and the yellow brought with your work to Handing In Day.

Coloured triplicate forms are available by sending s.a.e. to the Exhibitions Secretary.
